New Student Registration Information
In order for your child to be accepted into Saint Theresa Catholic School, the following forms, fees and documents must be received. The receipt of the registration will hold a place for your child. All fees are non-refundable except in the case where the school does not accept a student.
Please submit the following forms, fees and documentation:
1. Parent/Student Information Form.
2. Tuition Agreement Form with Fees.
3. Original Birth Certificate
4. Original Baptismal Certificate
5. Current Immunization Records:
  •   Hepatitis A is required for 3 years to 5 years
  •   2nd MMR is required according to State Regulation
  •   3rd Hepatitis B
  •   4th OPV is required according to State Regulation
  •   5th DTP is required according to State Regulation
  •   Varicella vaccine
  •   Tetanus booster if last vaccine was 10 years ago

  • 6. Bus Registration Form and Fee.
    7. Any court ordered custody/visitation directive
    8. $250 per child Registration Fee (activity/book/material/technology)
    9. $150 per family Capital Improvement Fee

    Once your fees and documents are submitted, we will arrange an interview with you and your child. (Kindergarten and first grade students must take a Gesell test.) During this interview, your child will take an entrance exam. The results of the test and the paperwork will be reviewed by the Admissions Committee, and you will be informed of our decision as quickly as possible.
